概述
鼠李糖乳杆菌( Lactobacillus rhamnosus)大多存在于人和动物的肠道内,细菌分类学属于乳杆菌属、鼠李糖亚种,为厌氧耐酸、不产芽孢的一种革兰氏阳性益生菌。它具有耐胃酸、耐胆汁盐、 耐多种抗生素等生物学特点。因此,它可以几乎100%活体进入人体肠道,而其他大部分益生菌种在进入肠道前就已经因胃酸和胆汁的作用而死亡。LGG®是鼠李糖亚种的一种菌株,属于全球著名的丹麦Chr.Hansen(中文:科汉森)益生菌公司拥有的品牌,于1985年从人体肠道中提取而来1。自1990年以来,LGG菌株已在全球范围内用作食品和膳食补充剂的成分。
LGG菌株在250多份描述人类科学研究的出版物中被引用。在所有调查鼠李糖乳杆菌的科学研究出版物中,超过75%使用了Chr.Hansen。LGG在各个健康领域进行了研究,包括新生儿2、早产儿3、儿童4、5、6 和孕妇7,8和成人及老年人9,10,且没有已知的安全问题。

LGG临床研究
自1990年以来,有关鼠李糖杆菌LGG®菌株的临床研究已发表250多篇,涉及多个健康方面。其中,研究最好的领域是消化、免疫和口腔,在婴儿、儿童和成人中观察到这些方面的健康益处。简要介绍如下:1.LGG涉及儿童的主要健康研究:
1.1. 稀便和水样便:
LGG可减少婴幼儿常见的稀便或水样便,缩短症状时间2,6,11,以及促进恢复大便粘稠度4,12。
1.2.与住院有关的健康状况
LGG可预防或改善住院相关的消化系统疾病5,以及降低住院后可能发生的呼吸道疾病感染5。
1.3.与呼吸系统健康相关的免疫挑战
LGG可降低上呼吸道感染发生率,缓解呼吸道不适症状、减少感染后持续时间等5,13。
2013 年进行的一项荟萃分析评估了LGG 降低儿童呼吸道感染风险的能力。纳入了四项随机对照试验,包括1,805名儿童,符合研究人员的纳入标准。LGG显著降低了儿童上呼吸道感染(URTI)、中耳炎感染和抗生素使用的发生率。此外,亚组分析发现LGG 能够明显减少一岁以上儿童的整体呼吸道感染13。
荟萃分析的一项关键研究是在克罗地亚进行的一项随机对照试验。该研究评估了日托中心的281名1至7岁儿童。孩子们被分成几组,接受添加了10亿 CFU 的LGG 发酵乳饮料或单独饮用不添加的乳饮料(对照)。孩子们在冬季期间每天服用他们各自的牛奶饮料,持续三个月。在LGG组中,只有43%的儿童发生URTI,而安慰剂组为68%(p<0.001),如下图所示。益生菌组的胃肠道感染也有所减少(p<0.08)。

与对照组相比,LGG组的儿童也经历了明显更短的疾病发作和更少的日托缺勤14。其他研究也证实了芬兰日托中心儿童URTI的减少15。
1.4.口腔健康
LGG可促进牙龈健康16,降低口腔中的有害菌水平17。
